Our client is seeking a Director of Case Management to lead and oversee the implementation of the case management program at the hospital level. This position is responsible for ensuring efficient care facilitation, utilization management, discharge planning, and case management services. The ideal candidate will provide leadership, supervision, and guidance to Case Managers and Social Workers while maintaining compliance with regulatory standards and optimizing patient care outcomes.
Responsibilities:
- Provide leadership, education, and supervision for Case Managers and Social Workers.
- Oversee documentation compliance to meet regulatory requirements.
- Collaborate with the CFO and Quality Department to develop quality improvement programs.
- Maintain skills in case management and utilization review to provide coverage as needed.
- Communicate with physicians to support patient care planning and appropriate bed placement.
- Manage personnel actions, including hiring, performance reviews, and scheduling.
- Facilitate daily multidisciplinary rounds to enhance patient care collaboration.
- Participate in discharge planning and ensure patients and families receive necessary resources.
- Engage in Utilization Review and Revenue Cycle committees.
- Ensure efficient use of clinical resources and appropriate resource allocation based on patient acuity.
- Maintain compliance with Joint Commission, CMS, and other regulatory standards.
- Perform other duties as assigned.
Must Haves:
- Education – Graduate of a Registered Nursing program.
- Experience – Minimum of two years of case management experience in utilization management, case management, or discharge planning.
- Licensure – Current RN license in the state of Iowa or a multistate license allowing work in Iowa.
Nice to Haves:
- Bachelor of Science in Nursing (BSN) preferred.
- Two to three years of previous management experience in a hospital setting.
- Iowa Mandatory Reporter – Child and Dependent Adult Abuse Certifications.
- Strong knowledge of payer requirements, Medicare, managed care, and case management principles.
- Effective leadership, communication, and collaboration skills.
Location: Ottumwa, IA
Salary: $93,272 – $125,900
